This form is designed to assist university-recognized groups in developing comprehensive safety plans for events held on campus. It also helps the university evaluate appropriate safety measures, event visibility, and coordination to ensure events are conducted safely and successfully.
For all University-sponsored events, regardless of whether they are held on or off campus, an assessment must be made as to whether the coverages described above are adequate to protect the University. If not, a separate event policy should be considered. These policies may be purchased by an affiliate (e.g., the University Foundation), or the University may work with FARM to broker a policy at the University’s expense. In general, an event policy is the best way to ensure that the University is fully protected from any potential claims or losses.
This form should also be completed when the event is:
- Anticipating more than 50 in attendance.
- There is a risk of injury.
- Examples of these events include 5Ks, tournaments (e.g., football, basketball, dodgeball, kickball), Pie a person, obstacle courses, aquatic events, protests, etc.)

Events requiring security, traffic control, or emergency access planning

Events that may impact parking, building access, or normal campus operations
- Emergency Management reserves the right to request the submission of a Safety Planning Form for any event.
Events do not include classes, department meetings and seminars, or performances and athletic practices under the auspices of the university as part of the ordinary operations of the university.
