Graduate students are expected to assume full responsibility for knowing graduate program procedures and regulations, which are located in the Graduate Catalog and in program handbooks.
Change to Graduation Application
This form is for students who want to update the following information on their graduation application: Diploma name, diploma mailing address, graduation date, cancel their application, and/or ceremony attendance. Access the form in MyÂ鶹ÊÓƵ by searching "graduation" in All Cards.
Graduate Course Repeat Request form (downloadable PDF)
Graduate students may repeat two “C” or “F” grades.
Request for Graduate Transfer Credit Review
This form is for graduate students who would like to transfer credits from another accredited institution or from another program within Â鶹ÊÓƵ. In MyÂ鶹ÊÓƵ, search "transfer" under All Cards and choose "Graduate: Pre-approval Form."
This iform is for graduate students who want to take more than the maximum allowed credits for one semester.
Request for Program Withdrawal - Graduate Students Only
For graduate students who want to withdraw from their current program (includes dual programs) if they want to change to a different Â鶹ÊÓƵ graduate program. Students who wish to withdraw from the semester or university should contact Megan McCue at mmccue@iup.edu.
